Online claim submission—hearing aid providers
Register or make changes to your account
If you are a provider and you have not registered for website access, please complete the two steps described below.
Step 1—apply for site access
Fill out the online form to request access to the secure web site. Once your site access is approved, we will email you the login ID you provided us with, as well as a temporary password.
Step 2—apply for direct deposit of funds
Fill out and print the direct deposit form, check it carefully, then sign and date it. Mail or fax your completed form to

Adding a new practitioner to your existing account
Please complete the Add a Practitioner online form if you’d like to add a new practitioner to your existing account for online direct billing.
Make changes to your existing account
Please complete the Request for Changes form under the following circumstances:
- You have changes to report (for example, you’ve had an address, phone number, email change). Please indicate what change has been made under “details of change requested” at the top of the form.
- You wish to switch to single sign on. Please note: single sign on only applies to offices with multiple provider practices. It allows providers to use a single login ID password to submit claims for products belonging to multiple provider types.
Step 3—add a practitioner to your account (for clinics only)
Fill out the online form to validate your practitioners. You will need to complete this step in
order to select the provider of service during time of claim entry.
