Submitting your claims

There are three main ways to submit your claims for reimbursement:

  • Mail in a completed, signed claim form to Alberta Blue Cross, 10009-108 St. NW, Edmonton, Alberta T5J 3C5. For your claim to be processed, original receipts and other supporting documentation must be attached.
  • Visit our web site for plan members to submit most types of claims if your plan allows this.
  • Download our My Benefits app for iPhone or Android devices and submit most types of claims quickly and easily through it.

Extended health claims

Prescription drugs

Depending on your plan design, there are two ways to pay for your prescription:

  1. Direct bill: If your benefit plan includes this feature, your pharmacist will be able to bill Alberta Blue Cross electronically for prescriptions. Simply show your Alberta Blue Cross identification card as proof of coverage when ordering your prescription, and your pharmacist will take care of submitting the claim for payment.
  2. Reimbursement: Prescription drugs may also be paid for directly and then claimed for reimbursement on a Health Services Claim Form.Remember to include your original receipts when mailing a paper form.

Or, providing your plan allows this feature, you can submit your claim online through the web site for plan members. Instead of mailing in a printed claim form (as described above), sign into the member web site and click on the “Submit a claim” link. Easy-to-follow steps will guide you through the online claim submission process.

Dental claims

There are three ways to arrange payment:

  1. your dentist may have arranged to submit claims electronically.
  2. make arrangements for "assignment" billing with your provider, by signing the top right hand corner of the Dental Claim/Treatment form.
    Payments will then go directly to your dental provider
  3. pay your dentist for services and submit the claim form to our office for reimbursement on a Dental claim form,

Spending accounts

Health Spending Account claims

  • Paper form: Mail in a completed, signed a Health Spending Account claim form to Alberta Blue Cross, 10009-108 St. NW, Edmonton, Alberta T5J 3C5. For your claim to be processed, original receipts and other supporting documentation must be attached.
  • Submit online: Providing your plan allows this feature, you can submit your claim online through the web site for plan members. Instead of mailing in a printed claim form (as described below), sign into the member web site and click on the “Submit a claim” link. Easy-to-follow steps will guide you through the online claim submission process.

Wellness/Taxable Spending Account claims
Note: Your wellness spending account may have a different name (for example, Flexible Account) but generally, it provides reimbursement for products and services which are not medically-related but rather, maintain and enhance wellness. The spending account provided your employer has a customized list of eligible expenses so it’s important to check them before claiming.

Use a customized spending account PDF claim form: This form is available by signing in to the member web site. Once inside the site, navigate to the “Forms” area to use the form named “Wellness Spending Account” that is pre-populated with your name and address and includes a list of expenses that are eligible under your specific plan. Mail your completed and signed form, along with relevant receipts to the Alberta Blue Cross address available on the form.

Life and disability claiming guides

The following guides will assist you in properly submitting disability claims based on your specific condition:

Direct bill for chiropractic, massage therapy, physiotherapy and vision claims

Your chiropractic, massage therapy, physiotherapy and optical provider can submit your claims online for you. When your provider uses this option, you will no longer need to pay out of pocket for the entire claim then wait to be reimbursed for the portion covered by your benefit plan. Your provider will receive payment directly and you will only pay for the portion not covered by your plan.

Note: The majority of claims can be processed online your provider’s office, but if your claim cannot, please pay for your services and then use a Health Services Claim form

Claiming procedures Claiming deadlines
Most plans outline a specific period of time in which allowable expenses must be submitted. Refer to your benefits booklet to determine how long you have to submit your expenses.
